Small Business Administration

Small Business Administration Applications must be submitted through the Upload function of the website, or via mail. The necessary forms for the loan application are found on the SBA Disaster Loan page: They are also available on this page.


You will need to complete all of the following forms:


  1. If a Sole Proprietorship: SBA Form 5C; If any other format (S-Corp, LLC, C-Corp, etc.): SBA Form 5

  2. SBA Form P-019

  3. SBA Form 413D for all owners

  4. IRS Form 4506-T

  5. 2019 Tax Returns for the business AND all owners. If those have not been filed yet, Profit & Loss Statements and Balance Sheets for 2019 and 2020 YTD.

  6. SBA Form 2202

  7. SBA Form 1368


These forms are the same as the original online application.


Once you have completed and signed all forms and have your 2019 tax filings or statements in hand, print them all off and scan them together as one file. It may be several pages long, but that is ok.

  • Once you have them combined, return to the link above, scroll halfway down and upload them to the UPLOAD BUSINESS FORMS box.

  • In the description box that appears, type KANSAS COVID-19 EIDL – YOURBUSINESSNAME.

  • Enter your email address in the next box and click UPLOAD.
